DHADHA Medical Laboratory Technologist Exam

Blood Bank questions for the DHA lab exam.

The Blood Bank section covers ABO and Rh grouping, antibody screening and identification, crossmatching, blood components and transfusion reactions. Candidates for the DHA lab exam are expected to resolve grouping discrepancies, select compatible components and follow safe pre-transfusion procedures from sample receipt to issue.

    Key facts to remember.

High-yield blood bank points to revise before the exam.

    1. 1

Group O RhD-negative red cells are the universal donor red cells, while group AB plasma is the universal donor plasma.

    2. 2

ABO antibodies are mainly IgM and react best at room temperature, whereas most clinically significant alloantibodies, such as anti-D, are IgG and react at 37°C.

    3. 3

The direct antiglobulin test (DAT) detects antibody or complement already bound to red cells in vivo, as in hemolytic disease of the newborn or hemolytic transfusion reactions.

    4. 4

Red cells are stored at 1-6°C, while platelet concentrates are stored at 20-24°C with continuous gentle agitation, typically for up to 5 days.

    5. 5

Anti-D immunoglobulin is given to RhD-negative women to prevent sensitization to RhD-positive fetal red cells.

    Questions, answered

Compare the forward (cell) grouping with the reverse (serum) grouping and identify which result does not fit the expected pattern. Common causes include weak or missing antibodies in newborns and elderly patients, rouleaux, cold autoantibodies and recent transfusion or stem cell transplantation. Resolve the discrepancy before issuing group-specific blood, and use group O red cells if transfusion cannot wait.

Know acute hemolytic reactions (usually ABO incompatibility), febrile non-hemolytic reactions, allergic and anaphylactic reactions, TRALI, TACO and delayed hemolytic reactions. For each, learn the typical timing, key signs and the first action, which is to stop the transfusion and keep the line open with normal saline. Be ready for questions on the laboratory investigation that follows, such as a clerical check, repeat grouping and a DAT.
