Questions, answered
Combine the history with sensibility tests (cold and electric pulp testing), percussion, palpation and radiographs, and give both a pulpal and a periapical diagnosis. A lack of response to cold suggests necrosis, but sensibility tests can be unreliable in recently traumatized teeth, heavily restored teeth and teeth with immature roots. Choose the treatment that matches the combined diagnosis.
Vital pulp therapy, such as direct pulp capping or partial or full pulpotomy, aims to preserve pulp vitality and is particularly valuable in teeth with immature roots. Calcium silicate cements such as MTA are generally preferred to calcium hydroxide because they give a more consistent dentin bridge and seal. Success depends on controlling bleeding, maintaining asepsis with rubber dam and placing a well-sealed coronal restoration.
