Hematology questions for the MOHAP lab exam.
Hematology in the MOHAP lab exam, set by the UAE Ministry of Health and Prevention, covers hematopoiesis, CBC interpretation, anemias, hemoglobinopathies, leukemias and hemostasis. Candidates should be able to connect laboratory findings to likely diagnoses and understand the principles and limitations of routine hematology tests.

Key facts to remember.
High-yield hematology points to revise before the exam.
- 1
Normal adult hemoglobin is mainly HbA (alpha2 beta2), with small amounts of HbA2 (alpha2 delta2) and HbF (alpha2 gamma2); a raised HbA2 supports beta-thalassemia trait.
- 2
The reticulocyte count reflects marrow red cell production: it rises after hemolysis or acute blood loss and is low in marrow failure or nutritional deficiency.
- 3
G6PD deficiency causes episodic oxidative hemolysis with Heinz bodies (on supravital staining) and bite cells, triggered by infections, certain drugs or fava beans.
- 4
The Philadelphia chromosome, t(9;22) producing the BCR-ABL1 fusion gene, is the hallmark of chronic myeloid leukemia.
- 5
The Westergren ESR rises with increased fibrinogen and other acute-phase proteins, which promote rouleaux formation and faster red cell sedimentation.

Learn the normal hemoglobin fractions and how each disorder changes them, for example a raised HbA2 in beta-thalassemia trait and the presence of HbS in sickle cell disease. Know the main methods used, including HPLC, hemoglobin electrophoresis and the sickle solubility test. Remember that coexisting iron deficiency can lower HbA2 and mask beta-thalassemia trait.
Both give microcytic, hypochromic red cells, but thalassemia trait usually shows a normal or raised RBC count with a disproportionately low MCV and a normal ferritin. Iron deficiency shows a low ferritin and often a raised RDW. A Mentzer index (MCV divided by RBC count) below 13 favors thalassemia trait and above 13 favors iron deficiency, but it is only a screening aid.
