Hematology questions for the SLLE lab exam.
Hematology in the SLLE covers blood cell development, automated and manual cell counts, red and white cell disorders, hemoglobinopathies, hematological malignancies and hemostasis. Candidates should be able to interpret CBCs and blood films and relate findings to diagnoses, including sickle cell disease and thalassemia, which are relatively common in the region.

Key facts to remember.
High-yield hematology points to revise before the exam.
- 1
Sickle hemoglobin results from substitution of valine for glutamic acid at position 6 of the beta-globin chain.
- 2
The corrected reticulocyte count adjusts for anemia: reticulocyte percentage x (patient hematocrit / normal hematocrit).
- 3
Acute lymphoblastic leukemia is the most common childhood leukemia, and its lymphoblasts are myeloperoxidase negative, unlike myeloblasts.
- 4
Smudge cells on a blood film are characteristic of chronic lymphocytic leukemia, which typically shows many small, mature-looking lymphocytes.
- 5
When nucleated red cells are counted as white cells, the WBC is corrected: corrected WBC = WBC x 100 / (100 + NRBC per 100 WBC).

Questions, answered
Organize them as acute versus chronic and myeloid versus lymphoid, then learn the blood film features, cytochemistry and key genetic markers of each. Examples include Auer rods and myeloperoxidase positivity in AML, t(15;17) in acute promyelocytic leukemia, t(9;22) in CML and smudge cells in CLL. Basic flow cytometry markers, such as CD19 for B cells and CD3 for T cells, are also worth knowing.
The sickle solubility test screens for HbS but cannot distinguish sickle cell trait from sickle cell disease, so HPLC or hemoglobin electrophoresis is needed to confirm. False negatives can occur in young infants, whose blood is still mainly HbF, and in severe anemia. Know the blood film features of sickle cell disease, including sickle cells, target cells and Howell-Jolly bodies from hyposplenism.
