SLLESLLE Laboratory Technologist/ Technician Exam

Clinical Chemistry questions for the SLLE lab exam.

Clinical Chemistry in the SLLE covers the measurement and interpretation of metabolites, electrolytes, enzymes, proteins, hormones and therapeutic drugs, together with method principles and quality control. Candidates are expected to interpret results, recognize sources of analytical and pre-analytical error and relate abnormal patterns to disease.

    Key facts to remember.

High-yield clinical chemistry points to revise before the exam.

    1. 1

Serum protein electrophoresis separates albumin, alpha-1, alpha-2, beta and gamma fractions; a narrow M-band, usually in the gamma region, suggests a monoclonal gammopathy such as myeloma.

    2. 2

About 40% of serum calcium is protein-bound, mainly to albumin, so total calcium must be interpreted with albumin; ionized calcium is the physiologically active form.

    3. 3

Indirect ion-selective electrodes can report falsely low sodium in severe hyperlipidemia or hyperproteinemia (pseudohyponatremia); direct ISE methods are not affected.

    4. 4

Raised amylase and lipase suggest acute pancreatitis, with lipase being more specific for the pancreas and staying raised longer.

    5. 5

For most drugs, therapeutic drug monitoring uses trough samples taken just before the next dose, unless a peak level is specifically required.

    10. Question 10

Which factor causes positive interference in both BCG and BCP albumin methods?

    Questions, answered

For each common method, learn the principle and one key interference: spectrophotometry (Beer-Lambert law), ion-selective electrodes, immunoassays (hook effect, heterophile antibodies) and electrophoresis. Questions may ask which method suits an analyte or why a result is unexpected. Linking each method to its limitations is more useful than memorizing instrument details.

Key causes are hemolysis (raised potassium, LDH and AST), lipemia (photometric interference and indirect ISE errors), icterus, delayed separation (falling glucose, rising potassium) and EDTA contamination (high potassium, low calcium). High-dose biotin supplements can interfere with streptavidin-biotin immunoassays, including some thyroid and troponin tests. When a result does not fit the clinical picture, consider the sample before the patient.
