    Key facts to remember.

High-yield immunology and serology points to revise before the exam.

    1. 1

Type I hypersensitivity is IgE-mediated with mast cell degranulation, whereas type IV is delayed and T-cell mediated, as in the tuberculin skin test.

    2. 2

Rheumatoid factor is an antibody, usually IgM, directed against the Fc portion of IgG; anti-CCP antibodies are more specific for rheumatoid arthritis.

    3. 3

In a sandwich ELISA the analyte is captured between two antibodies, so the color produced is directly proportional to analyte concentration.

    4. 4

Fourth-generation HIV screening assays detect both HIV-1/2 antibodies and p24 antigen, shortening the window period compared with antibody-only tests.

    5. 5

C-reactive protein is an acute-phase protein made by the liver that rises within hours of infection or inflammation.

    Questions, answered

For each type, learn the mechanism and one or two classic examples: type I (IgE, such as anaphylaxis and hay fever), type II (antibody against cell-bound antigen, such as hemolytic transfusion reactions), type III (immune complexes, such as serum sickness) and type IV (T cells, such as contact dermatitis). Questions usually describe a scenario and ask for the type. A one-page summary table works well.

Brucella antibodies are commonly screened with the Rose Bengal plate test and quantified with the standard tube agglutination test to give a titer. Brucella ELISA for IgM and IgG can help distinguish recent from longer-standing infection. Blood culture remains the definitive diagnosis, and suspected Brucella cultures must be handled with appropriate biosafety precautions.
